Yes, titles get lost because of how vBulletin works. For example, JMM, who has never been banned or post-restricted here in any way, lost his "PFA Bert Sugar" title. I never touched his account since giving him the title. It just vanished.
How does this happen? vBulletin has a flaw where auto-promotions of user title will override custom titles. So when someone went from "Member" to "Contributor" by reaching 500 posts, it would clobber their custom title.
Same with anyone whose access level was changed for any reason (such as a post restriction).
I later found that setting a custom title as "HTML allowed" (even if I don't include HTML in the title) fixes this problem, but I discovered it on my own.
I don't administrate forums in a passive-aggressive fashion. Never have, never will. If I am restricting or banning someone, I say so. I even announce when I ban someone from a thread.
